In order to be a part of the PhotoMedia Diaries team, you MUST be hired as an Independent Contractor. In order to be paid for your assignment as specified in each contract, you must complete the form. This information ONLY used for Federal Revenue Services Tax filings.

This form allows you to be hired by PhotoMedia Diaries LLC throughout the year. You will receive a contract for each project that you accept.
